It is required by the State of California, Department of Health Services, that all children must be properly immunized before entering school. Below is a table of the required immunizations your child needs to complete in order to attend school.
18 months through 4 years

1 Test with result (must be given within one year prior to entering school). A chest X-ray is required if positive. |
Varicella (Chicken Pox) |
1 Dose |
All immunizations must be properly dated with the month, day, and year. If your child is not completely immunized, he/she will not be allowed to enter school.
