1. All prescriptions must be in the original pharmacy
container, labeled with your child'sname, name of the medication, doctor's
name and dosage of medication. Medication that is listed as three times
a day will be given every eight hours.
2. Over-the-counter medication will only be given according
to the directions on the label. Please check the label. If the directions
state - under 6 years old consult your doctor - we cannot
administer it unless there is a note from your child's doctor stating
the dosage to be given. Over-the-counter medication will only be given
for three days, except when it is directed by a doctor in writing.
3. Parents must bring medication into the office. Do
not send it with your child.
4. A medical authorization slip must be filled out and
signed by a parent in order for medication to be dispensed.
5. If a child has a fever in the morning or the evening
before, he/she may not attend school. If your child is sent home with
a fever, he/she may not return to school the following day and must
be fever free for 24 hours - without the aid of Tylenol or Advil -
to be readmitted to school.
6. If your child is sent home with diarrhea and/or vomiting,
he/she may not return to school the following day and must be symptom
free for 24 hours before being readmitted to school.
7. Please remember to update your emergency cards whenever
you change jobs, cell phone numbers or home numbers. It is vital that
we be able to contact you at any time during the day.
8. Flu season is fast approaching. Getting plenty of
rest, eating healthy food, and by following the above rules, we shall
all endure the season with at least a healthy smile. |
