IMPORTANT NOTICE:
Please remember that ONLY parents and guardians may bring medication to the nurses's office for dispensing during the day. ALL medication - prescription and otherwise - must be turned into the nurse's office in it's original bottle by parents or guardians. Also, parents or guardians MUST complete an Authorization Form for Medication.
Please do not send children to the nurse's office to drop off medication. Nor should it be given to a teacher or TA. It cannot be dispensed without the proper form. 
All prescription medications must be in
the pharmacy container, properly labeled with
the name of the child, doctor's name, name
of the pharmacy, name of medication, dosage
and time medication is to be given. Medication
that is listed on the bottle 'three time a
day' will be given every eight hours.
Do not send sample bottles of medicine.
State law prohibits
nurses from making diagnoses or prescribing
treatment. However, please
feel free to ask about identification of health
conditions, nutrition information, or physical
activity recommendations for your child.
