To begin the admissions procedure for Pinecrest Woodland Hills Middle School, please print out, complete and submit the first three forms below.
Frequently during the year, class field trips to points of interest (museums, libraries, etc.) are conducted. Also, members of our sports teams travel to other schools to compete in afternoon athletics. In either case, and in all instances where students leave campus under the supervision of Pinecrest Schools, a signed Parent's Consent Form (Trip Slip) is required for each child in order to release them from school.
These Trip Slips are distributed a few days prior to the activity, and must be returned to school before the child may leave the campus. Details of each trip are given on the Trip Slip. If there are any additional questions, simply phone the office.
In the event of your having a Trip Slip or Consent Form
misplaced, we have provided blank forms for you to complete and send to
school. You
can call the school to get the information needed to complete the forms. They
are provided in Adobe PDF format.
Middle
School students may leave campus unescorted after the school day has ended
if there is a permission slip on file with the Middle School Office signed
by a parent or guardian. These forms are provided below in PDF format.
